There are a few forms to complete if you're a new client.  You can complete the following forms ahead of time and bring them to your first session or you can wait and complete them when you come.  You will need to print these forms and fill them in by hand.  Please click on the names at the bottom of this page.

First is a contract for services:

This form informs you of my privacy practices and the one at the end is where you sign saying that you have received it

• HIPAA

 This form allows me to bill your health insurance form for you:

If you would like me to communicate with others who know your child (for example, primary care physician, counselor, teacher, etc.), complete this form:

• Consent to Exchange Information Form

If the client is an adolescent with some expectations of privacy please use this form:

• Adolescent Informed Consent
